Dr. David Choate is a chiropractic physician who founded Midtown Medical Integration in 2022. Dr. Choate earned his Doctor of Chiropractic degree from Parker University, College of Chiropractic in 1987. With more than 35 years of experience in chiropractic care, Dr. Choate recognized the need for a comprehensive regenerative medical treatment center in Lakeland that offers therapeutic non-surgical pain management and relief services.
At Midtown Medical Integration, Dr. Choate focuses on holistic healing and restoration of injured or damaged joints, ligaments, tendons, and anatomical tissues without the use of steroids and narcotics or having to endure multiple surgical procedures. Through a multifaceted approach to alternative care, Dr. Choate assesses each patient’s specific needs and collaborates with his team of regenerative medical experts to determine a treatment plan that may include ultrasound-guided injections for joint pain, trigger point injections, platelet rich plasma (PRP) therapy, regenerative cellular tissue product injections, massage therapy, physical therapy, chiropractic care, and weight loss, to name a few.
In addition to owning and operating Midtown Medical Integration, Dr. Choate is a DOT certified medical examiner and a member of the Florida Chiropractic Association. He also provides FAA BasicMed flight physicals and enjoys giving back to his community through volunteerism. In the past, he has served as secretary and treasurer of the Polk County Chiropractic Society, an adjunct professor at Southeastern University in Lakeland, and a utilization review committee chairman of the Metroplex Neurospinal & Diagnostic Association in Dallas, Texas.
In his free time, Dr. Choate enjoys staying active in his local church, spending time with family, jogging, gardening, and working outside in the yard.

Danielle Rylott, MS, APRN, FNP-C
Midtown Medical Integration
Danielle Rylott is a certified advanced practice registered nurse (APRN) and family nurse practitioner (FNP-C) who joined Midtown Medical Integration in 2022. She is a graduate of Polk State College, where she earned her Associate and Bachelor of Science degrees in Nursing. In 2019, Danielle earned her Master of Science degree in Nursing from the University of South Florida.
Prior to joining the team at Midtown Medical Integration, Danielle worked as an advanced practice registered nurse for Trinity Medical Group, a registered nurse at Lakeland Regional Health, and a nurse technician at Winter Haven Hospital. Additionally, she was a recipient of the Winter Haven Hospital Partnership Scholarship in 2013.
At Midtown Medical Integration, Danielle’s clinical duties include utilizing integrative and regenerative medicine to treat injuries and provide non-surgical relief from chronic and degenerative conditions without the use of steroids, opioids, or other narcotic pain management medications. Danielle is certified in ultrasound-guided injections for joint pain, trigger point injections, platelet rich plasma (PRP) therapy, and regenerative cellular tissue product injections.
When she is not at the office, Danielle enjoys hiking in the mountains and spending time with her family.
